- Mechanism of Action: The precise mechanism through which risperidone treats schizophrenia is not fully understood. It is believed to act by blocking dopamine Type 2 (D2) and serotonin Type 2 (5HT2) receptors. Its therapeutic effects are attributed to both risperidone and its metabolite, 9-hydroxyrisperidone (paliperidone).

- Side Effects: Includes increased mortality in elderly patients with dementia-related psychosis, cerebrovascular events, neuroleptic malignant syndrome, tardive dyskinesia, metabolic changes (hyperglycemia, dyslipidemia, weight gain), hyperprolactinemia, orthostatic hypotension, falls, blood disorders, cognitive and motor impairment, seizures, dysphagia, priapism, and disruption of temperature regulation. Caution in patients with Phenylketonuria.
- Drug Interactions: Significant interactions with fluoxetine, paroxetine, carbamazepine, ranitidine, cimetidine, erythromycin, amitriptyline, levodopa, ketoconazole, verapamil, furosemide, paliperidone, dopamine agonists, rifampicin, and anticholinergics.
Indication: Risperidone is indicated for the treatment of schizophrenia.
Contraindications: Avoid in patients with known hypersensitivity to risperidone, paliperidone, or any formulation excipients.
Precautions: Elderly patients with dementia-related psychosis treated with antipsychotics are at increased risk of mortality.
Warnings:
- Neuroleptic Malignant Syndrome (NMS): A potentially fatal complication reported with antipsychotic use.
- Tardive Dyskinesia: Involuntary, potentially irreversible movements associated with long-term use of antipsychotics.
- Metabolic Changes: Atypical antipsychotics may increase cardiovascular/cerebrovascular risk through metabolic effects like hyperglycemia, dyslipidemia, and weight gain.
