Generic Name: Omeprazole
Used For: Gastroesophageal Reflux Disease (GERD)
How it Works: Omeprazole inhibits gastric acid secretion by blocking the H+/K+ ATPase enzyme system, also known as the proton pump, in the stomach lining. This action reduces both basal and stimulated acid production.

Dosage: Omeprazole
Side Effects:
- Common: Headache, diarrhea, constipation, abdominal pain, nausea/vomiting, flatulence.
- Uncommon: Dizziness, paraesthesia, somnolence, insomnia, vertigo, increased liver enzymes, rash, pruritis, urticaria, malaise.
- Rare: Reversible mental confusion, agitation, aggression, depression, hallucinations (mainly in severely ill patients), gynecomastia.
Drug Interactions:
- Ketoconazole, itraconazole, diazepam, citalopram, imipramine, clomipramine, phenytoin, atazanavir, digoxin, clopidogrel, posaconazole, erlotinib, saquinavir, tacrolimus, methotrexate, clarithromycin, CYP2C19 or CYP3A4 inhibitors (e.g., rifampicin, St. John’s wort).
Indications:
- Treatment of erosive reflux esophagitis
- Long-term management of healed esophagitis to prevent relapse
- Symptomatic treatment of GERD
When Not to Use:
- Hypersensitivity to any component of the formulation or to substituted benzimidazoles.
- Concurrent use with nelfinavir.
Risek Capsules Precautions:
- Omeprazole may reduce vitamin B12 absorption. Consider this in patients with reduced body stores or risk factors for reduced B12 absorption on long-term therapy.
Risek Capsules Warnings:
- Exclude the possibility of gastric malignancy if a gastric ulcer is suspected, as treatment may alleviate symptoms and delay diagnosis.
- Long-term use of proton pump inhibitors, especially in high doses, may increase the risk of hip, wrist, and spine fractures, particularly in the elderly or those with other risk factors.
- Consider dose reduction in patients with impaired hepatic function due to increased bioavailability and half-life.
